When you use an assessment, care-request form, or provider application, we may receive the details you choose to submit, including contact, location, care-request, and provider-organization information.
How we use information
We use submitted information to provide requested guidance, maintain private review queues, respond to inquiries, and operate the service. We do not publish family care requests.
Provider sharing
Felicitas does not refer, introduce, or transmit a family’s submitted contact and care-request information to a participating provider without the applicable consent. Declining provider-sharing consent does not block general guidance.
